Now you have me worried.
I rinse thoroughly, fill with a gallon of hot water and PBW, shake vigorously, run 1/2 through the tap, rinse and repeat with hot Starsan.
What am I missing?

Quilt secured with bungee cords at the bottom. Burner base wrapped 80% around with aluminum foil <- *Important*
No - I usually mash at 149°-151°F for 90 minutes and mash out to 165°F
I stir at 30 minutes (-0°F) and 60 minutes (-1°2°F). My mash volume is about 12 gallons...
Started out making Stone IPA clones with 1968 - made great beer but I wanted a drier finish.
Ran with 1272 for many batches - highly recommended.
Last few have been with San Diego Super - cleaner, drier.
The other important piece that is not being discussed here is water volume. Find a good BIAB calculator, measure accurately and save your data. Efficiency cannot be accurately calculated without accurate volume measurement, especially strike water needed, and volume of ambient wort.
Other things to consider:
Do a full volume mash and make sure the grain is not constricted.
Mash 90 minutes
Stir at least every 20-30 minutes
Make sure your pH is not too high
