Should look for when fermentation is slowing down and the krausen has drops out. You want primary to be mostly completed, after most of the fermentable sugars have been consumed.

I use a thermowell with an airlock. Went to a science store and bought a solid bung and drilled two holes in it. Works great.

From my experience, the volume doesn't play that big of a role. With more water, you might get a slightly higher extraction rate but you will just be diluting the beer that much more. Think of it as a coffee press, less water the stronger it is.
