Gallon Batch problem

I've just made my second gallon batch which came out flat after three weeks of bottle conditioning. I had added 1 oz. of sugar solution before bottling, but both batches were flat.

Should I add more sugar than 1/5 of the standard 5 oz sugar measurement? I'm sure this is probably an easy fix but I'm kind of stumped.
 
What were the conditions of the bottle storage while they were supposed to be carbonating? Were they refrigerated or kept cool? They should have 3 weeks at 70 F. or above and you have to have capped them tightly. If you are using PET bottles, the lids are known to be a problem and you have to tighten them more than you would expect.
