Cider Frementing slowed down

It slowed down to about one bubble every 2 to 3 mins. Is it done fermenting can it be racked or bottled?

My Recipe was. 1/2 gallon of unpasterized farm fresh cider. 2 cups of brown sugar, 3tbs of cinniamon & the yeast was US-05 if that information helps.

best way to check is with a hydrometer. if it's down around 1.000 or below, you know it's nearly finished fermenting out.
