What's wrong with my beer?

I am not new to home brewing but I am not an expert by any means. This is the first batch I have had any trouble with. As soon as I open my beer it foams out of the bottle and does not stop. Over the last month it has lessened (slightly). Any input would be greatly appreciated!!

Sounds Like its over carbonated... But I'm not sure
