I want to brew a ginger lime wheat ale (or belgian witbier) all-grain. I was inspired by Boulevard Radler beer. Aiming for 5% ABV with citrusy hop aroma (maybe Sorachi & Motueka hops).

When is ideal to incorporate ginger and lime? During mash, boil, or fermentation?

As far as lime goes, I use the zest just as you would dry hopping. Squirt a little star San in it and dump it in.

Ginger I have only used in the boil. Last 10-15 min.
Normally if it's your first time using ginger, how ever much you think you need..use half.
This goes from just enough to rip your face off really quick
 
If you have a juicer you could juice the ginger and use some with the priming sugar or towards the end of the fermentation...but it is incredibly potent so use sparingly. I would start with 2-4oz whole ginger, juice that (you will get a lot less), and then maybe dose a glass of beer with a few drops to get an idea of how it will taste/strength.
