I have a lid for my bottling bucket & keep it covered at all times as well. Gotta keep those unwanted buggers out of my new beer, ya know? I also dip my caps in Starsan and cap the bottles as I go, crimping them when I get a dozen or so. No problems doing it this way. I got a lacto infection in my BB ale pail & pitched it. It didn't have a lid seal anyway. Replaced my auto siphon & racking tubes, etc as well. The bottling wand cleaned & sanitized is still fine though. I got a Midwest shorty bucket with lid seal to replace the ale pail. No problems since.
Yep. I do an assembly line when bottling, I still crimp the caps all in one go, but I now fill, stick a sanitized cap on top, then move to the next bottle. This (and the bucket lid) may be the key piece... though if so, it seems odd I had brewed for so long without this issue in the past.
For sure. Always better do over do on safety than wish you had. I just don't like the thought of nasty's settling into my beer when it's so close to being safe & cozy in it's bottles.

I typically use the lid from my StarSan bucket while bottling if I don't have another one handy.
As for caps, I'll just dump a handful in a small bowl of StarSan, fill 6 bottles, then just pull caps out of the StarSan right on top of the bottles and crimp 6 at a time.
