When I did my first batch it was a simple extract with added hops at the end of the boil. I used a clean pot but did not sanitize it nor the spoon. To remove and squeeze out the hop bag I used a sort of tongs that I only washed but didn't sanitize.
My next batch was a little more complicated with more hops and grains which I removed with my spoon. At the end I used two spoons and my hands to remove and squeeze out excess liquid/flavor after removing my kettle from the heat to remove the hop bags.
Should this equipment be sanitized or is the heat enough to keep it relatively safe?

Everything that touches the boil must be clean.
Everything that touches the beer after being removed from heat must be clean and sanitized.
