Random bottles foaming over, am I priming correctly?

I prime with corn sugar when bottling. I use a brewing calculator to get the right amount. But sometimes, I get a few bottles that foam over when opened, while others in the batch don't.

I want to see if I am doing something wrong in my bottling process. I measure the sugar, pour it unto a very small amount of near boiling hot water, dissolve, and pour into the bottom of my empty bottling bucket. Then I siphon the beer into the bucket, figuring this action alone mixes in the priming sugar.I do sometimes get a little bit of.the yeast cake into the bottling bucket too.

Is it possible I'm either not mixing priming sugar thoroughly, or getting too much yeast or yeast clumps into some of the bottles?
 
Could be that the priming sugar is not mixed thoroughly. This would most likely result in the first bottles filled being over primed. Is it the first bottles that foam, or is random? If it is random bottles that foam, it may be an infection from dirty bottles.

I number the first six bottles filled, and the last six to check for problems with the priming solution not being well mixed in the bottling bucket.
 
I always give my priming sugar solution a gentle stir after racking the beer on top. How do you clean your bottles? PBW and star san? The gushers could be coming from improperly cleaned bottles.
 
I transfer my beers to separate container for cold crash. I mix bottling syrup and gelatin finings in at this point and cold crash for 3 or 4 days before bottling. The results are very consistent.
