Well, here's my problem.

I seem to have issues transporting my bottles of beer. If I leave a bottle here I can drink it anytime. If I take it with me to someone's house for some reason it almost always ends up being over carbonated within a few days.

For example: I drove across the state with 3 bottles. One cold and two warm. All three of them ended up pouring with too much foam. Several months ago I lost about 24 bottles to the same fate.

The beer should be finished fermenting (1-2 weeks in primary, the same in secondary) so I don't think that's the problem, we're not over carbonating by any means (4.5-5oz per 5/gal batch).

Any suggestions?
 
How long are you letting them sit before you crack them open? I find that most of my homebrews need to sit for 36+ hours without movement in order to pour perfectly.

Also, a fast temperature shift might be what's causing this if you're cooling them down quickly.
 
Yep, bottles need to be chilled and not moved for (ideally IMO) 48 hours to absorb the CO2.

Also, 1-2 weeks doesn't mean anything...Gravity readings are the ONLY way to know the beer is done fermenting. I've had a moderate gravity stout sit a month, get bottled, then explode after 5 weeks because it wasn't done and I didn't rely on my gravity readings.

depending on style and if you actually had 5 gals to bottle, you could be over carbing. a 5 gallon kit is really only gonna get you 4.5 gals into bottles after racking to secondary and again to the bottle bucket.
