I have been having issues with priming here latly. I use 1.5 cups of pale ale malt to prime 5 gals of beer. I disolve this in a water solution and then pitch into the beer. Stir thoughly and bottle. Two batchs ago some bottles over carbonated and turn into a volcano when opened. Then the last pale ale batch didn't carbonat at all and is flat.

Any process change suggestions to insure a more consistent carbonation.
 
I always use the recommended 5 oz. of dextrose (corn sugar) for a 5 gal. batch. I never have never had any problems.
 
+1 to corn sugar. Buy in bulk and it lasts a long time. I always stick to in between 3/4 and 7/8 of a cup boiled in 1 pint of water for five gallons, works great for me.
