Newly bottled IPA fizzes when opened

Just opened a tester of my bottled 2 weeks ago IPA, and as soon as I popped the cap it acted as if shook up, about half of the contents spilling out the top if the bottle. I had a Christmas ale act similar, so I imagine it's something that I've done.

Any suggestions on how to save the batch?

a few questions:
How long had the bottle sat in the fridge?
How much priming sugar did you add?
How much beer were you priming?
How well did you mix the priming sugar?
 
The bottles are not yet in the fridge, and have been in the bottle for 2 weeks. I bought the corn sugar "pack" from Midwest for the 5 gallon batch, I think it was 1/2 cup maybe?
