Hi Gang,
So, I made an all grain batch yesterday and was off by 24 points (hit 1.054, calculated 1.078) on my gravity. I had the same thing happen to me two weeks ago when i brewed. These were around my 25th time brewing all grain and I have not run in to this before. The only change in my process is that I got my grains for the last two batches from a new LHBS. I checked the crush on the grains for the second batch to make sure they were actually crushed well and they looked fine to me.

So, a little about my brew from yesterday.
Going for a IIPA.
13.5 # us 2-row
1 # white wheat
1# Crystal
1# Cara-pils
I use one of the converted 10 gallon coolers as my MLT.
I mashed at about 151-152 for 90 minutes with about 6 gallons of water. Temp dropped to about 147 during that time.
Batch sparged with another 3-4 gallons of 172 degree water.
I didn't take any hydrometer readings which I do plan on doing next time (sorry!).
Boiled down to a little over five gallons and took the reading at around 72 degrees for a 60 degree rated hydrometer. I know that could make it off by a couple points but not 24 points.
The only other thing that changed is that my MLT was not quite as high when lautering (maybe a foot lower) but I can't see how that would make a difference. It took maybe a half hour to lauter.

So, I am really baffled as I have hit my OG pretty closely on all the previous batches I had done and I just can't figure out what went wrong on the last two batches.

Any ideas? Oh, I checked my thermometer in ice water and near boiling water along with a different digital thermometer and they were both really close to each other and at where they should be for the cold and hot temps.

The main thing that jumps out to me is you used a new LHBS and they crushed, correct? That would be the first and easiest place to look. Go back to your old LHBS or try an online supplier and brew the same beer again.

It could be something else, but this one seems like a slam dunk. And if it is the crush, shame on that LHBS. That would be a really sh!ty crush to have you lose 24pts.
 
Thanks dobe12! That was what I was thinking but the grains looked ok to me. I am going to try another shop and brew the same thing and see if it happens again. Really frustrating to be so off on the gravity. I ended up adding a pound of DME that I had laying around to at least bump up the gravity a little. Hopefully the beer comes out drinkable!
