Foamy Beer After Few Month Bottle Conditioning

Hey All,

New to the forum here, this is my first post. I've been brewing extracts for several months and have done a total of 6 batches so far. To date, the beer has turned out great. I usually have a six pack or two that last a couple months in the bottle. Once I pop these open that have been sitting, all that comes out is foam and the beer is virtually up drinkable. This only happens with beer that sits for 4 or 5 months. I have no idea what is causing this. Could it be fluctuating temperatures?
 
3 possible thing comes to my mind. Over carbonation, the beer has not finished fermentation, and the least likely is infection.

The beer is OK within the first few months, though.
 
I have had similar results with an ipa. I'd leave my bottles out in my closet and stick a few in the fridge for a week or so. But then I had some that I left in the fridge and forgot about them. After a few months it was way more carbed than if I had it in there a week. I since toned down my priming sugar and I leave beer in the fridge for three weeks before opening one so it's at full carbonation. If it's not finished fermenting then it will produce more carbonation as time goes on. So like I said it could be the issue.

Yeah, I leave the bottles in my bedroom so the continued fermentation could be the issue.
