That could be an issue...I try to leave the warts behind! ;)

You changed a lot in that batch, and I agree that squeezing is probably not the cause of issues. I do question if grinding grain down to flour is really the way to go. I get pretty good results setting my roller mill to a credit card thickness.
 
If you are blaming the potato masher then you are really grasping for straws...IMO
I agree. Tannin extraction is almost always the result of pH being too high (above 6) and temps over about 170°F. High temp alone won't extract tannins, otherwise every decocted brew would be a tannin bomb (oh tannin bomb, on tannin bomb... sorry.)

There are commercial breweries that use pneumatic or hydraulic filter presses to squeeze the wort out of the spent grain. You can't squeeze anywhere near a hard as they squeeze. They wouldn't do this if squeezing could extract tannins.
