Adding coffee to beer?

I have an extract porter kit that I would like to add coffee to. Does anyone have any advice on how to do this? For example:

What do you do with the beans - grind them, slightly crush them or leave them whole?

When do you add the beans - steep them with the grains or put them in with boil or in the secondary?

How much would you use for a 5 gallon batch?

Any help is appreciated.
