100 percent efficiency?

For some reason when calculating my efficiency I boiled total of 6.25 gllons of wort and collected 6 Gallons total with 4.5 lbs flaked wheat and 4.5 lbs uk pale malt and says my efficency is 100 percent? Any Clue with this?
 
What was your pre-boil gravity? Post-boil?

You started with 6.25 gallons and only boiled off a quart?
 
According to Beersmith, if you used 4.5# each Flaked Wheat and Pale Malt you would need to end up with 6 gallons at 1.053 OG in the primary.

I suspect one of three things:
Incorrect weight of grain, or
Incorrect final measure, or
Incorrect hydrometer reading.
 
Final Gravity was 1.042 and post boil was 6.25 and only got it down to 6 gallons. Usually I have more boil off but guess not. Total of 9 lbs of grain.
 
That means you had an efficiency of about 79%. Good, but not 100%.

.042 / (((4.5 * .035) + (4.5 * .036)) / 6) = 79.2%
