Recommended Hole Size for Homemade Mash/Lauter Tun

I made a home made lauter tun with a 5 gallon cooler and some cpvc pipe. I dry fitted everything together and it fits just right. The liquid flows through well but so does a lot of the grain. My thoughts were many but primarily these: My holes in my "false bottom" are too big and allowing too much grain through, My grain was ground too fine or I didn't pour my wort back over my grains enough times to create the filter I need. Could it be any or all of these? What should I look at first? I immediately thought of the holes because they are actually "holes" and not a mesh that would certainly keep out any grains.
 
The "holes" should probably be slots about .04"~.050" wide... What size drill did you put the holes in with?
