I only have us-05 or s-04 yeast which should I sub?

either would be fine, but I would suggest s-04.

This sounds awesome. If making an extract version or mini-mash, any recos on how much DME to use? Thx!

You would need a mini mash at least if you planned to use the flaked oats. Here's a conversion to partial mash from Beersmith:

6 lbs 6.7 oz Light Extract (8.0 SRM) Extract 56.62 %
2 lbs 3.1 oz Pale Malt (2 Row) US (2.0 SRM) Grain 20.72 %
13.1 oz Oats, Flaked (1.0 SRM) Grain 7.71 %
13.0 oz Caramel/Crystal Malt - 60L (60.0 SRM) Grain 6.78 %
12.0 oz Chocolate Malt (350.0 SRM) Grain 6.24 %
3.7 oz Black (Patent) Malt (500.0 SRM) Grain 1.93 %
1.25 oz Fuggles [4.50 %] (60 min) Hops 19.2 IBU
0.25 oz Fuggles [4.50 %] (15 min) Hops 1.9 IBU
0.25 oz Fuggles [4.50 %] (5 min) Hops 0.8 IBU
0.25 oz Fuggles [4.50 %] (0 min) Hops -
1.00 items Whirlfloc Tablet (Boil 15.0 min) Misc
5.33 oz Malto-Dextrine (Boil 20.0 min) Misc

How many pounds of blueberries to use? I'm thinking about 6. Also, do you use "fresh" berries then freeze or frozen berries from the store? Thanks for your help. Planning to brew next weekend.

6 lbs is a good amount.

You can use store frozen, or fresh. But if using fresh, freeze them before using for best results. I have used both kinds to good effect.
