I forgot how to mash

Okay, bear with me here. Is the following typical for single temp infusion?

1. Mash in
2. Mash out
3. Collect run off
4. Sparge
5. Collect sparge
6. Brew

I have been
1. Mash in
2. Mash out
3. Collect run off
4. Brew

My last three mashes have been 63,57 & 61%. Pretty damn consistent. Is the first order what most people use?

yep.. that's what most use..
 
Except for "Brew" should be "Boil" either one is fine.

Single Sparge
No Sparge

If you hit your target volume and gravity your made the beer you were aiming for.
 
It depends if you batch or fly sparge I guess.

I batch sparge (much to the chagrin of my fly sparging neighbor) and have my rig dialed in to 73%. Still possibly low, but works consistently which is the important thing.

I use a variation of your 1st listing there.
-Mash in at about 1.45 qts/lb
-stir and mash for an hour (also recirc with a pump)
-add a small amount of hot water to raise the temp an thin the mash a bit.
-sparge 1
-add hot water again, recirc to clarify.
-sparge 2
-boil.

That 2nd rinse should get you more gravity from your grains...all other things remaining equal (mash temp, boil duration, etc).
