haha Ive done that too with a wing capper. After you refill a bottle like 10 times I think some get a little weak in the neck
 
I've had a problem reusing Lagunitas 22's and shearing off the top. I think certain bottles are thinner in the neck. Did it make a crunch sound before it popped (I've noticed this too).
 
But notice that all of the beer still seems to be in the bottle. To this I say......well done:cross:

I bottled for the first time last night and, while I had nothing like this happen, there was a bit of a learning curve.
 
Yea, I've been here many times. This is why I'll usually bring out 10% more bottles to get sanitized on bottling day. I've lost as many as two during one 50x bottle session. Oh well. Just one more that I'll have to drink and delabel someday I guess :)
