Try using the bags available in HBS stores for containing your hops. I've used them for some time with great results. Makes kind of a hops "tea bag." Cleanup is a snap.
Just taking a shot here, but you don't let your wort cool for a long time before pitching your yeast do you?

Distilled water? Is that a normal practice for homebrew? I thought that the minerals in drinking water were essential to proper brew chemistry. Hey, if you're getting good results with it, great. But, I've never heard of using it and am curious.
