What did I do wrong on my partial mash?

I am a newer brewer and attempted my second partial mash brew. First one hit perfect. Anyways this was my first attempted at a Pale ale. I performed a 60 min mash at 155 degrees
5 Lbs of American pale ale
4 oz of crystal 40
4 oz of biscuit
2 oz of carapils
Mashed for 60 mins with a total of 7.5 qts of water. Sparged with 5 qts of 168 degree water.
Brought to a boil removed from heat added lbs of light lme
brought back to a boil added 1 oz of simcoe hops at 60 mins
willimette .5 oz at 40 mins
Cascade .5 at 30 mins
.5 cascade and .5 willimette at 10 mins
Cooled water to approx 75 degrees and topped off fermenter to 5.5 gallons with fresh water.
Current temperature of was was 67 and reading OG of 1.048 accorded to brewers friend it was supposed to be an OG of 1.060 at 75% but looks like I got around 45-50% efficiency. I don't understand what I screwed up any advice out there thanks. Looks like I am going to have a light Pale Ale then I wanted.
 
Are you sure you got your top-off water thoroughly mixed in? The sweet wort will not readily mix in without some effort.
The "heavy" wort will want to lay on the bottom.
 
Acidrain thanks a lot man I just checked it again at its right on. I sanitized the crap out of my stir spoon and sure enough after I mixed it I got 1.060 why didn't I think of that. Thanks man.
