Hello,
I have a 2 tap keezer that I have had for over a year. I started having an issue with one of the taps- after running 4 or 5 kegs, one of the taps is always flat and the other is fine. It is a Keg Connections set up with a 2 outlet manifold, always set at 11 psi. I have tried 2 different kegs with different beers in them, no luck. The last keg has been on for months to see if it would carb up and its still flat and the other side is carbed perfectly. I checked connections and still have plenty of CO2, so I assume no leaks- ideas?? Thanks for any and all help!!!

Can you dispense beer from the keg that's always flat?
If you pop the relief valve on the flat keg, is there any pressure in it?

Check to make sure that when you pull the relieve valve to make sure co2 comes out. I'm guessing is that there won't be any pressure. You can check for leaks by spraying starsan and water solution on all connections and check for bubbles. I always find that if I don't use keg lube on the large o-ring co2 will leak out.

I have pin locks, so I checked and the keg does have pressure, also checked for leaks on the keg seals, no bubbling. After releasing some CO2 from the keg it pours with some foam but flat (poured flat/no head earlier today)- might release most of CO2 off keg and see what it's like in a day or 2.

Did you clear the headspace when you kegged the beer? If not, perhaps there's air trapped in there, which would prevent the keg from carbing as the C02 would not be directly contacting it. It would oxidize instead, I'm assuming.
