Depends. Are we talking the original version? It can be, but sometimes isn't. The more modern version? Usually is, but doesn't necessarily have to be. So let's just call it "usually".

That'd depend on the brewer's interpretation, but I'd say no, not normally. And if it does contain wheat, it shouldn't be a major character.

The vagueness is narrowed a bit by the semi-compliance with the Reinheitsgebot, although I'm having some trouble wrapping my head around that.

Doesn't require a special fermenter.

As for the Reinheitsgebot compliance, I don't think I'm giving too much away to say that sugar is often used for a boost. Prohibited under the original, but allowed in ales under the modern (at least, that's my recollection of the law).
