    A few things. It might be fine and just taking it's time. How old was the wyeast? Did the wyeast puff up after your smacked it? What temp are you fermenting at? If you are really worried you can always smack another pack and pitch it.

    From my experience, you need at least 5 psi to get the lid to seal. I've used an aeration stone to carb up stouts with beer gas. I'd say start at 5psi and then bump it over time if that is what you are looking for.

    For a buck a piece, I have a few extra. I broke one the same way you did, broken one by stepping on it, and tossed one too hard into my sink. Keep a few extra :)

    There are a lot of threads of pin vs ball lock, check 'em out. I went ball lock for 2 reasons, I can fit 6 in my fridge exactly (probably only 4 of pin) and I wanted the pressure release pull.

    Airlock foam through

    Sometimes my fermentation goes wacky like that too, usually it's a good thing. I would certainly take that airlock off, clean it in pbw/starsan, and put it back. If you are nervous about taking it off starsan some tinfoil and quickly swap it on while you clean the airlock.
