Should I keep the wort covered as it boils?

No. Boiling uncovered allows volatile flavors and chemicals to boil off. Always leave the cover off!
 
Just to qualify that, I sometimes will put the lid half on to get up to a boil faster but then I take it off. The key is that you don't want any of the stuff that condenses on the lid dripping back into the wort.
