Pressure barrel question

Hi, I'm looking to buy a pressure barrel, and I've seen a couple with wide necks for easy cleaning. They seem to say that they use the pressure within the barrel to dispense the beer, but then mention using Co2 capsules. What's the deal here, isn't the pressure from secondary fermentation enough ? Once you remove a few pints you'll need a CO2 top up.
