New to kegging cider - need help

There seems to be different info on this in doing a search. So I'm going to ask what is the latest up to date info on this? I've got 15 gallons of hard cider that I want to back sweeten with apple concentrate and keg. What are the CORRECT ratios of:

Campden Tabs
Potassium Sorbate
Apple Concentrate to add?

The batches that I have are all over 6 months old.

K-meta (campden) and K-sorbate should have instructions on the packet. It's usually something like one tab for every gallon and half a teaspoon of sorbate for every gallon, I'm not sure though because I almost never use it. The concentrate would be to taste, so keep adding till it's where you like it.
