Do you put the grains in the water and THEN start to heat or do you get the temp correct and then put the grains in.
 
Heat the water to your strike temp and then mix in the grain. Be prepared to adjust with some direct heat (while stirring) or some cold water if you over or undershoot but don't react immediately. Temp equilization does take at least 5 minutes after it's mixed.
