Just brewed my 8th batch and OG came in at 1.040 (expecting 1.060)
5 Gal BK/BIAB 2.5 gal batch
I am calling this a partial mash. These are the details.
Recipe:
3.00# LME
1.00# American Pale 2 Row
.50# Crystal 60
.50# Carapils

.50oz Galena 60 min
.25oz Galena 30 min
.50oz Centennial 15 min
.25 tsp Irish Moss
1/2 Pkg US05

Mashed grain @152F for 60 min. Rinsed thru colander. Added LME, brought to boil and got good hot break. Boiled Hops as per recipe. Cooled to 70F (20min) and put in fermenter (3 gal BB) Added 3/4 gal water to top off. Stirred and aerated thoroughlyand took OG reading.

How many gallons were you shooing for/wound up with? Your numbers aren't showing up right in Beersmith.
 
I don't think you stirred thoroughly enough, and/or you ended up with more than 2.5 gal.
Just the LME would give you a gravity of 1.044.

Yeah, I get 1043 just with the LME.

Have you checked your hydrometer? That's the only way this makes sense if you're really at 2.5 gallons and really have 3# of LME in there regardless of the other stuff.
