Dispensing from keg - what happened to the carbonation!?

I am posting after scouring through this forum looking for answers. We have a fermented beverage we are using Torpedo kegs to force carbonate. To make things clearer, I'm listing our high level steps:
  • Batch, strain, and chill ferment
  • Add to corny keg (Torpedo kegs 1.5 gallon and 5 gallon)
  • Force carb @ 35PSI (for about 1 week)
  • When ready to bottle: keep kegs and line cool during bottling
  • Turn off gas, and vent keg
  • Lower regulator to ~5-10PSI
  • Dispense beverage using 10 ft of line
  • Dispense beverage into "waste bucket" to prime and cool the lines
  • Dispense beverage into glass bottles using a counter pressure filler and crown cap
We are finding that the beverage comes out flat. When we open up the keg to check on the beverage with a ladle, the beverage is perfectly carbonated the way we want it. How can we dispense the beverage without losing all that carbonation!? Any advice would be helpful - thank you in advance!
 
What kind of beverage is it? If it's alcoholic, what is the %ABV?
What temperature is the beverage in the keg when you're trying to fill the bottles? You said "cool" but it should be nearly freezing cold.
When you say it's flat, is it foaming a lot during the fill?
