CO2 - threaded connectors, leaking issue

Im new to brewing and kegging... And am having an issue with my recently purchased setup.

The CO2 regulator has threaded connections, and my CO2 hose has threaded swivel nuts. The connections at both the regulator and keg ends are leaking... Any ideas why?
 
Because you didn't insert the nylon washers inside the swivel nuts?

If so, you should be able to tighten the couplings with a wrench and stop any leaks at the couplers. If you're still loosing gas there, make sure the hose clamps on the swivel nuts are tight. Not much else to it...

I think the general rule with MFLs is to go finger tight, then barely snug it with a wrench. Overtighten and you'll just crack the washer.
