Calculating efficiency

I've seen a lot of threads on tips and ways to maximise the efficiency of your wort from the mash, but I'm still a bit confused as to how you would come up with that percentage. I've been trying to find a topic that goes over how to do this, but I can't seem to find one. Is there a topic or anything that goes over the measurements, variables and math needed to calculate out your efficiency accurately? Or maybe a spreadsheet or online calculator or something?

It's basically a calculation of the max sugar that could be extracted from the grains compared to how much you were able to extract during the mash. You convert it to PPG (points per gallon) factoring in water volumes and then divide one by the other to get your efficiency.

I use this spread sheet for all my brews to track how well I am controlling my processes.

Is there nowhere in this spreadsheet that lets you take into account the type of malt? It would be nice not relying on having an internet connection but it seems like this might not be the most accurate.
 
The top section is where you enter the specifics of each malt in your grain bill. You enter the weight of the grist, its potential, and its water content. Those variables are then used to calculate the potential sugars, and thereby the efficiencies of your various processes.
