Ok. So first the recipe

Only change is it used caramel 15 and only got 5.25 gal. Gravity was 1.062 which I figured at 74.66%

Is that right?

OG for the recipe is 1.054 and they list their brewhouse efficiency at 70%, so without doing the calcs, that sounds about right. You did good! No issues. Lately I have been having to reduce the grains when I do other people's recipes because I have been getting 75 - 85% efficiency. Just part of the learning process of dialing in your system.
 
A quick estimate:

12 lbs at roughly 36 points per lb would be ~432 points if you got 100% (in 5.25 gallons, that would be ~1.082).

You got 1.062 in 5.25 gals. Total points = 5.25 x.62 = 325 points.

Efficiency = 325 / 432 = 0.75, or 75%

Yep, you are in the right ball-park. My 432 (100% efficiency) number is only an estimate, so my number is not accurate.
