    bs2 calculates mash efficiency based on brewhouse efficiecny and kettle / chiller trub losses. If you need to change your mash efficiency, then you need to adjust one of those two variables.

    The numbers look good. Conversion is in a typical range at 90-94%. The first set is a bit uncertain though, as mash eff % is lower than brewhouse. Brewhouse should always be lower than mash, as the only difference is kettle losses. Most likely your volume measurement was off slightly.

    Double crushing is worthwhile if the initial crush is inadequate. You can only get so much conversion though, and most decent crushes yield 90-95% conversion efficiency, so if you're already in that range, then double crushing is unlikely to benefit much. Double crushing is not some magic...

    Measure the conversion efficiency next time, it's a better metric for grain crush, since mash/brewhouse also involves sparging, equipment losses, and how much grain is in the recipe. Conversion efficiency removes all those variabels.
