    Am I really understanding decoction correctly?!

    To avoid hot spots, you just have to stir. I know it sounds flippant and simplistic, but that's just the way it is. Just like mashing in, you gotta keep things moving to help spread the heat around. Each heating method will result in hot spots, but thorough mixing helps to mitigate that.
