I just went through this myself. I "think" it may have been a loose connection. You may want to regularly check (1/year ?) all the screw terminal connections and make sure that they are tight.

Why not replace just the O-rings? And maybe add an intertap spring?
As long as you don't need the plastic bearing cup(6) it should be easily rebuildable. The dash number of the o-rings listed below.

If you do a low profile coupler, It adds about 1.75" to the height of the keg. And you can convert it to use ball locks.

That's true but you'd have to allow for that to clear whatever you have your "kettle" on.
In my case, I didn't want to hack into my SS table that my kettle sits on. That's why I have it plumbed to go threw the skirt on the keg. I can rest it on a table without any ill effects.
Isn't the whole point of a bottom draining kettle to be able to "flush" everything out the bottom?
Use a 2" tri-clamp to cap off the end. That way it's removable.
