I use 10 to 13 lbs of grain. My favorite recipe uses 10 lbs of 2 row, 1 lb Crystal L120 and 0.5 lb Chocolate. Yummy

I wish that I had started with all grain.
Equipment was my constraining factor.
If you can afford an all grain setup then go for it.
If you can't then do extract.
Both can make very good beer

I think the best way would be to use a steam coil. Basically, the opposite of a wort chiller. Steam heats the coil and transfers the heat to the liquid in the tank. You would need a boiler to drive the coil. Pressurized steam is much hotter than boiling water.
