A voile curtain is cheap and works well. I didn't sew it but just use binder clips to hold it in place on my kettle. I also fold it in half to get double the filtering.
I like to dunk sparge into two gallons of water for 10 minutes. I've got a 7.5 gallon kettle, and have done a 12 pound mash using this technique. My efficiencies have been in the 75-80% range.

I just use a stainless steel vegetable steamer for mashing. It doesn't cover the entire bottom of my pot, but I make sure the direct flame is low so i'm not directly heating the edges of the kettle with a flame when I need to heat up a few degrees to adjust temp or mash out. I haven't had any...
For the bag I use a voile curtain. I bought a sewn bag from Austin Homebrew Supply for $7, but it didn't fit around the lip of my kettle very well and it wasn't fine enough to filter as well as I would like. The curtain works really well with some binder clips to hold it in place.
Just brewed my first AG today using BIAB. I missed my gravity by 1 point which is way better than I could have expected. It's going to be a holiday beer. I just wanted to say thanks to everyone in here for all the great tips!
I pulled a gallon off of a dubbel I brewed about three weeks ago and racked it on top of 1.5 pounds of frozen raspberries to keep SWMBO happy. I have no idea how it will taste, but it smells great.
