Basically it is a sanke tap that has had the gas-in check valve removed and a special fitting consisting of an 1/4" mpt adjustable pressure relief valve, 1/4" fpt brass "tee", a 1/4" mpt pressure gauge, and a 1/4" mpt to tailpiece fitting I had machined out of nylon. The machined fitting fits through a beer thread wingnut and is attached to the 1/4" fpt brass "tee". The beer-out port on the tap is pictured with a 1975 penny as a stopper secured with a beer thread wingnut and a rubber gasket. I now simply attach a tasting/testing line and spigot (not pictured) while I am fermenting, then un-tap when I am ready to carbonate while still at fermenting temperature. I re-tap only once crash cooling has the beer down to 35*F. This is where I adjust having too much pressure/carbonation prior to transfering to a serving keg.
