Made a yeast starter a month and a 1/2 ago and it has been in my fridge ever since. Just didn't have time to brew. Should it still be good?

Its probably had some cell loss but it should still have some viable yeast cells. You could always make another starter with it to ensure good health of the yeast.

No. If it still seems good (no infection or terrible smell etc.) you can decant any "beer" left on top and use the slurry for a new starter. Check the viability and size of the starter you would need to make with it using an online starter calculator like Mrmalty.
