I was brewing a sweet/milk stout today, and after I put my wort into the carboy, I tossed my yeast into my bucket o' starsan and popped the bung into the top of the carboy to aerate it for a minute while the yeast packet sanitized. Well, apparently I either wasn't paying attention or I used too much force, and I pushed the bung into the neck of the carboy. Thankfully, I was able to get it out with a sanitized butter knife and screwdriver, so, hopefully, it won't be an issue. We'll see!
