Default Wort Aeration

Do you need to still aerate the wort if you've made a good starter, or is this step only necessary when pitching a non-started yeast?

You still need to aerate.

Is shaking then good enough for the wort if you've aerated the starter well?

I think so. Shaking is actually a pretty good method of aeration. I get great (and clean) fermentation and that's all I've ever done.
