Just recently completed my sixth batch of an all grain cream ale.

Using BeerSmith it said my original gravity should be 1.051. Using a refractometer I had an OG of 1.053.

Considering the equipment I use and my lack of experience I'm pretty happy with that reading.

How far off is too far though? Are we talking +/- of .005 ok? or more like .001?

2 points, I consider that spot on! I think if you're within 5 points either way, you will be fine (just more or less alcohol if it ferments fully). You get farther than that, you probably start changing the beer profile and the bitterness level, but will let more experienced people then me get into more detail on that.
