I just use a simple excel spreadsheet. I keep:

Brew Name
Date Brewed
OG
Yeast
Fermentation Temp
Date: Secondary
FG
ABV
Date Kegged
notes

The notes section is where I describe certain techniques used, different ingredients off my standard recipe, alternate hops, etc etc.
