Need Help Creating Recipe for Armored Fist CDA

I'm trying to create a clone of Armored Fist (NW/Imperial/Double?) CDA by Boneyard Beer in collaboration with Three Floyds. I've never actually created a recipe before and am not sure how to do it. I figured HB Talk would be the best place to turn.

I was able to get a list of ingredients from the brewer but now need to get the quantities correct.

Here are more details.

Brewers Description: Imperial Cascadian is the NW style of this brew. This is a very well balanced, extremely hoppy black ale. It is very reminiscent of a winter ale.

Terminal gravity: about 1.018
Color: very dark brown, not black, with ruby highlights.
IBU: 80 (very hop-forward)
ABV: 10%
Malt profile: roasty, slightly chocolatey, and telling of deeply caramelized barley which implements a stone fruit, datey notion.
Flavor: Initial very hoppy flavor with a malty after-taste.

Grain: midnight wheat, caramel Munich, and Munich.

Hops: Sorachi Ace, Citra, Cascades

I had this for the first time last night and loved it! The Boneyard website doesn't really give much useful information anymore.
