Hand tightening the regulator adaptor to the CO2 tank is not sufficient!!! I bought a new tank 2 weeks ago and left town after I hooked it up to my cornie. Came home to an empty tank and uncarbonated beer. All the CO2 had leaked out while I was gone!! I initially thought I had a regulator malfunction as there was a horredous leak when I hooked up a new tank, but after I tightened down the adapter with the vice-grip I was good to go.....
 
Hi,
I highly recommend doing a leak check on all fittings. Get a small bowl of water, add a little detergent, and then gently sprinkle it over all fittings in your system.

If you have a leak it will begin to bubble.
