At what SG do you use a starter?

Prior to my last two batches, I had almost always used a starter, but my last two batches were only 1.052 and 1.062 SG's and used Safe Ale 04 and 05 so I didn't bother.

The fermentation on the 1.062 ended kinda early, should I do starters for anything over 1.060?

At what gravity do you consider doing a starter?

Are starters even nessessary for dry yeast?
 
I have never used a starter for dry yeast. I've used more than one packet for higher gravity brews as per Mr. MALTY though.

No starter for dry yeast, an ALWAYS as starter for any White Labs or Wyyeast, mainly because I have to build up one tube or smack pack to split across 3 batches, but also its just a good idea for liquid yeasts.
 
I have been trained to make a starter for anything over 60...
