how do pros fill/wash kegs?

Just wondering - anyone been on a tour or seen how this works? I know there are 'keg washers' but I'm wondering what the simplest, cheapest way smaller companies get the beer from fermenters to kegs for distribution.

i guess i'm wondering if the beer is carbonate prior to filling or during the fill and how one knows whether the carbonation is 'right' inside the keg.
 
My kegs have all come NASTY on the outside.

What is the best method to clean the outside? Being stainless steel, they should hold up to a lot.

Car wash, maybe? Then, go over the really bad spots with a green pad?
 
Gator Grit pads (lowes) and a angle grinder or drill. Then a wipe down with StarSan solution and a weeks "rest" to allow for re-passivation.
